Questions to take to a clinic

Get names that can be checked

Keep the person treating you, the service arranging the visit and the pharmacy on separate lines in your notes.

Check the person’s licence under their own name

Write the name of whoever will assess you in full, beside their licence details and role. Ask that person to name the state board responsible for their licence record.

The doctor’s state medical board has a public record; use it to check their current licence and any action recorded against that doctor.

For board websites, start with the Federation of State Medical Boards. The list also covers doctors with DO after their names.

This directory covers doctors; if a nurse or another kind of health professional will treat you, ask which board holds their record.

Ask it this way

Which board record matches the person who will assess me?

Check the pharmacy by its own name

An online care service may leave the identity of the pharmacy making its medicine unclear.

Name the pharmacy that will make the medicine and the place that will send it. Use the pharmacy’s full name for the record check.

The drug agency’s name is Food and Drug Administration; FDA is its short form.

The FDA’s state lookup page leads to pharmacy licence searches.

Ask it this way

Which pharmacy record applies where I will receive the medicine?

Read the limits of a clean record

If the board’s public record lists no action against a doctor, it still gives no rating of the doctor’s care.

Match the name and role, then bring the care questions to the visit. A record check cannot answer why this plan fits your needs.

Ask it this way

Who will answer questions after the visit, and how can I reach that person?
